Consumer Health Data Privacy Notice

Effective date: October 2, 2026

Masters Spine Center, LLC, doing business as Masters Spine Institute.

This Notice is provided under the Washington My Health My Data Act and applies to consumer health data that is not protected health information under HIPAA. It is separate from our general Privacy Policy by design, and it is limited to what this law requires.

It applies to you if you are a Washington resident, or if your consumer health data is collected while you are in Washington. We provide the same rights to residents of Nevada and Connecticut.

Categories of consumer health data we collect, and why

Symptoms and condition information. Neck, arm, and hand symptoms, how long you have had them, prior imaging, prior treatment, and prior surgery. Collected so a clinician can determine whether cervical disc replacement may be appropriate for you, and so we can respond to your inquiry.

Diagnostic imaging. MRI or other studies you choose to send us. Collected so a clinician can assess your cervical spine.

Health care you are seeking. The fact that you contacted a spine surgery practice, and the consultation or service you asked about. Collected to respond to you and to arrange the consultation you requested.

Location precise enough to identify an attempt to obtain health services. The state you tell us you live in, and the general location derived from your IP address. Collected to determine whether you would need to travel for care and to route your inquiry to the right staff member.

We collect and process these categories only to provide the consultation, assessment, or information you asked us for, and to comply with law. We do not use them for advertising.

Sources we collect it from

  • Directly from you, through the candidacy questionnaire, the chat, the imaging upload link, email, phone, and forms on our website
  • Automatically from your device, limited to IP-derived general location
  • From a health care provider or imaging facility, when you ask us to obtain records on your behalf

Categories of consumer health data we share

We share the categories listed above with the parties named below, only to deliver the service you requested.

We do not sell consumer health data. We have never sold consumer health data, and we do not collect it for that purpose.

Categories of third parties and affiliates we share it with

  • Cloud hosting and database providers, which store the data so our team can access it
  • Encrypted file storage providers, which hold imaging you upload
  • Communication providers, which deliver email and messages between you and our team
  • Our treating physicians and clinical staff, and, where you direct it, a physician or facility you name
  • Professional advisors and regulators, where the law requires disclosure

We do not share consumer health data with advertising networks, data brokers, social media platforms, or analytics providers.

Your rights

You have the right to:

  • Confirm whether we are collecting, sharing, or selling your consumer health data, and to access that data, including a list of all third parties and affiliates with whom we have shared it and an active contact for each
  • Withdraw consent to our collection and sharing of your consumer health data
  • Delete your consumer health data

To exercise any of these rights, contact us at privacy@mastersofspine.com or (833) 674-3627. We will respond within 45 days, and will tell you if we need a permitted extension.

If we deny your request, you may appeal by replying to our response or writing to the address below. We will respond to an appeal within 45 days and will explain our reasoning. If we deny your appeal, you may contact the Washington State Attorney General at www.atg.wa.gov/file-complaint.
